Clinician Ordering Process: How to Request Honor Alert for a Veteran

Honor Alert Services are issued through a medical prescription and coordinated through established VA clinical and prosthetics workflows. No separate vendor forms are required.

Step-by-Step Ordering Process

Use the standard VA clinical consult and prosthetics workflow to prescribe the most appropriate Liberty device for each veteran based on clinical need.

Step 1: Clinical Assessment

The process begins during a clinical assessment with the veteran’s VA physician or authorized provider.

During this evaluation, the provider determines whether a personal emergency response system (PERS/mPERS) is clinically appropriate based on the veteran’s health status, mobility, fall risk, and living situation.

Step 2: Device Selection

If the veteran is deemed eligible, the provider selects the most appropriate Honor Alert Service type, such as:

  • Standard Service mobile Alert Button Pendant
  • Mobile Alert Button Service with Fall Detection
  • Wrist-Worn Liberty Service

Service selection is based on clinical judgment and patient needs.

Step 3: Clinical Consult & Prescription

The provider documents the recommendation and writes a consult for the Honor Alert device within the VA system.

This consult serves as the official authorization for the Liberty Service alert button.

Note: No Honor Alert-specific forms are required. All orders are issued via medical consult.

Step 4: Prosthetics Review & Processing

The consult is routed to VA Prosthetics, where the order is reviewed and approved according to standard VA procurement procedures.

Step 5: Order Fulfillment & Shipment

Once approved, VA Prosthetics submits the order to Honor Alert for processing.

Honor Alert then:

  • Configures the prescribed Liberty Service
  • Activates monitoring services
  • Ships the device directly to the veteran

Step 6: Veteran Activation & Support

Upon receipt, the veteran receives instructions for activation and use.

Honor Alert provides ongoing 24/7 monitoring and support, ensuring the veteran has immediate access to help when needed.
